After a patient is enrolled into our Remote Patient Monitoring (RPM), our Care Team continuously ensures patients are educated on their condition and can empowers them to be advocates of their own health.  Reporting to our Director of Retention, you will manage a dynamic, diverse & growing team of Care Coordinators, while further defining patient care and compliance strategies to support business goals.  As a Care Team Coordinator Supervisor, you'll use your experience to hire, coach, and develop a team of new and established Care Coordinators, to meet defined metrics for compliance, retention and timely care.  You'll also work cross functionally with teams in sales, billing, and customer success to drive company initiatives and deliver the best care for each and every patient. #LI-RemoteCarbon Health is a leading national healthcare provider with a mission to make high-quality healthcare accessible to everyone.
